Intake
If you are interested in receiving services at The Douglas Center and explore possible programs and services, our Intake Coordinators are here to start that process.
Here are the steps:
- You can schedule a tour of The Douglas Center
- Complete the intake application process
- Please allow time for your application to be reviewed and processed
- Once the determination is made, a trial for the applicant will be scheduled at The Douglas center for 3 to 5 days and may be extended if more time is needed to assess.
Eligibility
- Community Day services for adults who are 18+
- Individuals with intellectual/developmental disabilities as their primary diagnosis
- Completed high school and /or in a transition program (day program services are provided after completion and not simultaneously with other school programs)
- Have funding available through the Medicaid waiver program or are interested in paying privately for the services.
- Commitment to attend the program at least 3 days a week
Funding
- PUNS: PUNS (Prioritization of Urgency of Need for Services) is a statewide database that families and individuals with intellectual/developmental disabilities can apply for. PUNS database helps the State to identify and plan for the individual’s needs. Please Register with PUNS as soon as you are able and eligible to apply.
- PUNS is available for anyone who is seeking services after high school or when they turn 18 years of age. The state of Illinois Department of Human Services provides funding for participating in community day services, medical needs, homecare, etc. The Douglas Center is a unique and specialized program that accepts the State funding received through PUNS as a funding option.

However, If you don’t have PUNS, you have the option of private pay.
- Private Pay: For individuals and families currently on the PUNS waiting list, or do not plan on receiving PUNS benefits, The Douglas Center accepts private pay options for all services and programs at The Douglas Center
