Case Management
At The Douglas Center we have individual case management, which means that each participant is assigned a case manager. The participant and the case manager build a rapport due to an open door policy and a consistent presence. The case manager is accessible, approachable and committed to their care. Case Managers are in contact with family members, residential staff, partnering agencies and other integral members of a participant’s care team, to help coordinate care. Our Case Managers advocate for participants needs to ensure they are able to have the best experience possible.
